Registration Information - Read and complete both the Member
Registration form and the Class Enrollment form carefully. Your signature is required in three places on this form.
- Be
sure to include the name of the participant who will be taking the class and his or her age. The instructors need this information.
- Sign the Policy Acceptance form indicating that you have read and understand the G.I.F.T. policies listed on this website
and that you will abide by them.
- Complete the Medical Release information for your child(ren).
- Sign the
Insurance Waiver form.
- The yearly registration fee is due upon submission of your registration form.
This non-refundable fee will be applied to administration and start-up costs. Registration fees are: $45 per member family.
- Please
make your check payable to "Godly Influenced Fine Arts Training" or "G.I.F.T.".
- Mail
your completed registration form and payment to:

Once
you are registered, you may add a class at any time without any late fees. The first month's tuition payments
for all classes and any additional teacher supply fees are due on the first day of classes September 22, 2009. Tuition
payment for each subsequent month is required on the first class day of each month. Classes will run a total of 28 weeks
unless otherwise noted. Please note that these classes are tentative and subject to change. Our final schedule will
depend upon enrollment. Classes that do not meet the minimum enrollment may not be offered. G.I.F.T. is not an accredited
school. Your signature is required on the registration form indicating that you have read and understand the policies
on this website and agree to adhere to them. If you have a serious concern about a policy, contact the G.I.F.T. board. THE
G.I.F.T. BOARD CANNOT BE HELD LIABLE FOR ANY DECISIONS OR INJURIES, INCLUDING THOSE RESULTING FROM TEACHER-LED ACTIVITIES.
